# Add a note

MCP tool: `sysaid_write → add_note`

Access: write. Confirm: waits for the user's OK before it runs.

Returns the note ID and whether it is internal or shared. Notes can notify users and groups your admin set up for "Notes are added", and an @mention in the text emails the person named. A shared note reaches the requester. By default you confirm each call; you can switch that off for this tool.

## Parameters

| Name | Type | Required | Description |
|---|---|---|---|
| `record_id` | integer | yes | Service record ID from search_service_records. IDs are unique across incidents, requests, problems and changes. |
| `body` | string | yes | Text of the note, 5,000 characters at most, no emojis. |
| `share_with_requester` | boolean | no | true shows the note to the request user. A shared note cannot be made internal again. Default false. |
